Systems Approach for Diverse Care

Submit responses in a Word document. Imagine you are the healthcare worker for three diverse patients diagnosed with type 2 diabetes.  How would you design a plan of care using a system’s approach and cultural competence for each patient?  Choose one or two different variables for each patient, such as gender, age, race/ethnicity, income or education, and factor that into your patient-centered plan of care.  Present three different patients, each with a different plan of care, in your assignment.  So your assignment should contain three separate plans of care.

For example, one patient you could write about might be an elderly Asian woman.  In designing her plan of care you’d need to think about: Are there age-related variables to consider?  Potentially.  (Is she able to understand clearly?  Does she have any other illnesses correlated with advanced age?)  Are there racial/ethnic or cultural variables to think about?  Possibly.  (For example, is language a barrier?)  And lastly her gender could be something to look at when designing a plan.  Do you need to tailor your approach to her based on the fact that she is a woman?  In writing up her plan of care you’d want to think about all of these things.
