Purpose

You are to obtain a lifestyle and health practices profile using yourself as the client, and then practice analyzing the data to formulate a health promotion nursing diagnosis.

Complete the Life and Health Practices Profile, using yourself as the client. Please document your responses professionally, as you would in a client's actual health record. Provide enough information to have answered the questions completely. For clarity, write full sentences in your documentation. To receive full credit, all questions must be addressed, except for those indicated as optional. If a question does not apply to you, please indicate this in the findings as "N/A".
Once you have completed the profile, type your answers to the three questions listed under the form. APA format is not a requirement, however, please write in paragraph form when appropriate. Sentence structure, spelling, and grammar are important.
You are required to complete the form using the productivity tools required by Chamberlain University, which is Microsoft Office Word 2013 (or later version), or Windows and Office 2011 (or later version) for MAC. You must save the file in the ".docx" format. Do NOT save as Word Pad. Select an Option to Complete this Assignment
There are 3 options for creating your Visual Teaching Tool. Be sure to choose a format that makes sense for your selected topic, population, and setting. For example, if will be teaching adults at a health fair, then an educational brochure would be an appropriate choice. You must use the Patient Teaching Plan you have developed in this course to create your Visual Teaching Tool.
Option #1 - Power Point
Directions:
Create a 6-8 slide Power Point presentation for your selected population setting.
The goal of this Power Point Presentation is to address the three learning outcomes you developed in the Patient Teaching Plan. Once the learner has viewed your Power Point, all three of the learning objectives should have been met.
For Example:

Under the Education option, select Education Brochure. This will provide you with a blank tri-fold brochure template. You can change the design, insert graphics, and create text as you wish.

This form of education can be helpful for patient populations with low-literacy levels, or to simplify a complex topic. Perform a web search to learn more about infographics, view examples, and understand how they are utilized.

You will be able to start an infographic, save it to your account, and edit it later.
When your infographic is complete, you will need to download and save to your computer in order to submit to Canvas for grading. Select the PNG (recommended) saving option.

Physical Examination Video Demonstration assignment is due Sunday end of Week 6 at 11:59 MT.
Directions
Create a video demonstrating a complete physical examination on a volunteer adult (18 years of age or older) participant. Due to the important nature of this assignment, recording the video on a non-human participant will result in an assignment failure. Videos longer than 15 minutes will be subject to a deduction in points.

Perform the physical examination in the order that is written on this outline. (This is meant to be a general physical examination. Skills that are not included on the outline do not need to be in your video.)
You may record your video in Kaltura (available to you in the course) or you may use your own phone or other digital camera device. ALL videos must be uploaded to Kaltura in the course shell for submission.

Please note that there are specific publishing settings that need to be correct to ensure privacy. You will need a stethoscope and a pen light to complete this examination.
Adult participant must remain covered at all times for privacy purposes. Suggested attire for adult participant: tank top and shorts. Instructor must be able to have a clear view of stethoscope placement on the torso. Auscultating over a shirt is acceptable for the purposes of this video. It is suggested that the student wear nursing scrubs and/or a lab coat. Business casual dress is also acceptable.
Video should be filmed in a private area with minimal distractions to enhance the evaluator’s ability to hear and see clearly.
You may not use prompts, notes, or take notes during this assessment. Doing so will result in a point deduction.
State your findings within the video.
To begin the video:
Student scans area with camera to show that notes are not being used (i.e., taped to walls, on a table)
Student states in front of camera: “My name is [First and Last Name of Student] and this is my physical examination assignment for NR 305.”
Participant states in front of camera: “My name is [First and Last Name of Adult Participant] and I give my permission for this assessment to be recorded and submitted to Chamberlain College of Nursing for grading purposes.”
