Overview
The purpose of this assignment is for you to complete a comprehensive psychiatric mental health assessment of an adult/older adult as practice for entry into PMHNP clinical practice and to continue the growth in your interviewing skills throughout the term.
Instructions

This should not be a patient you have encountered in your work but, instead, a family member or a friend. Your assessment should be comprehensive, and you should refer to course texts to inform items for inclusion in your assessment. Week 5 – Self-Injurious Client
Case Study: You are seeing a current client for a follow-up session with new developments of self-harm. This is not an initial interview. For this session, focus on the psychodynamic therapy modality to engage with the teen.
Psychodynamic psychotherapy – This type of therapy helps individuals explore issues lurking at the subconscious or unconscious level that often stem from negative childhood experiences and painful memories.
A 17-year-old female patient named Stella Jess, who recently broke up with her boyfriend, reveals she has been cutting her arms. While rolling up her sleeves to display the cuts, she expresses suicidal ideation. She has a suicidal plan, overdosing on her depression medication (Prozac) and alcohol, but says she doubts she would ever actually attempt to take her life. She reports no serious previous attempts. Her history includes some neglect from her parents when she was a younger child, and she now lives with her grandmother. Her mother is still involved in her life, but not on a daily basis. She is an A student, does not miss school, and states she has a group of close friends with whom she enjoys spending free time.

Contact Your Partner
Identify a time that both of you can meet online to record your video. Allot about 60 minutes for discussion with your partner and the recording of the video.

Video Recordings Need to Have the Following Components
The recording should show both you and the client during the entire session.
Both of your faces and body language should be shown.
Attire should be appropriate and professional.
Set up the room as if it were a therapy office (that is, chairs; a quiet, private space).
The recording needs to be clear and audible. Ensure there is no background noise when you are recording.
The lighting of the room needs to be appropriate; that is, it should illuminate the faces of the client and the therapist.
After the Video Recording
After your recording is complete, follow these instructions to create an unlisted YouTube video. You will need the YouTube video URL for this assignment. Submit your YouTube video URL. The best way to do this is to copy and paste the hyperlink URL for the YouTube video into a Word document. Your faculty will access your video via the link. Do not upload a video file (mp4).
Then, go to your Reflective Journal (after you have posted the video) to respond to the assigned journal questions and reflect on your interaction with the client.

Individual Adult Interview and Psychotherapy Treatment Plan
Ms. Alana, a 24-year-old married female Muslim client was referred for psychological intervention in the Psychiatry Outpatient Department. She was assigned to the present therapist and was diagnosed as depression by the psychiatrist. She and her husband would like to build a new life not associated with her past. In the assessment sessions she presented her problems along with history. Her problems are presented in the following summary in clustered fashion according to different areas of functioning.
The client believed that she was suffering from psychological illness. The client complained lack of concentration, lack of self-confidence, and indecisiveness. She also complained of depressed mood, feeling of guilt, lack of pleasure, anger and hopelessness. She felt irritability and fear. She avoids social gathering, friends and sometimes occasionally she used to cry. The client complained of headache, palpitation. She also complained that family members usually irritate her especially eldest brother. Her dress up, appearance and behavior appeared to be culturally appropriate. At the initial interview she spoke willingly about her problems. She was well motivated and interested to work collaboratively with therapist.
Exploration of history revealed that the client was in a middle-class family of a rural area with three brothers and three sisters. Her father is 57 year old and he was a small business man. Her mother is a 45 year old house wife. From her childhood she experienced that the relationship between her parents was not good. The eldest son of their family maintains everything of the whole family. Her eldest brother was very dominating. She had to lead her life as to his liking. She was the last issue of her parents. Though she was meritorious student fromchildhood she was always underestimated instead of being encouraged. The senior most brothers always used to apply pressure on her for studies. They were not happy with the results she obtained. During any bad occurrence in her family if she protested, she had been termed as “disobedient”. She likes reading story, listening to music and reciting poetry which are not supported by her elder brother. Her brother doesn’t even like her writing skills. She was physically tortured several times for doing these. She was sexually abused for several times. At the age of five or six years old, some of her playmates abused her. When she was in class seven her cousin tried the same way. During college life one of her uncles tried to abuse her also. She couldn’t tell these to her family with a fear of receiving disbelief of the family. When she was 15years, she had an affair with a boy. Then due to misunderstanding that broke up. When she was in college she again got involved with a boy only to pass time with that boy. Now she is having third affair. She is a graduate student. Since having all these she thinks that if she had got family support enough, there wouldn’t be so many problems. There was no history of psychiatric problem in her childhood and adolescence.
She is recently married and expecting her first child. She reports her husband is patient and caring and understands her past as a block in their relationship. She reports being hopeful for the future since she has been in America for 5 years now and wants a better life for her children. She currently works as an assistant advocate at the local college in the cultural and community center helping minority women navigate the college experience.
In clinical interview the client was asked the reason for referral, why she sought for help and how long the main complaint had persisted, when did the problem first occur, what was the subsequent development in her life (occupation, living with parents, at school), what were the impairments that have been produced by the her difficulties, how have she and others coped with the problem, what her belief about the problem, what was the attitude to her difficulties, what was her cognitive functioning, what was her prevailing mood, what was her background history, early development history, occupational and educational history, sexual history and what previous psychiatric, psychological or medical help she had taken. The client asked to find out and list up her main problems. Thought diary was applied to assess situation specific negative automatic thoughts (NATs) and corresponding emotion, physiological changes and behavior for the client. It was administered to identify the NATs about the social situation and the relation to changes in emotion, physical reaction, and behavior
Therapist conducted both type of measurement, subjective & objective measurement. In assessment session client mentioned her overall problems severity at ‘100’ point on (0-100) rating scale. For objective measure Depression scale was used to assess the severity of depression. The highest possible score of 30 items form of depression scale was 150 and the lowest possible score was 30. Higher score on the scale indicates high level of depression and lower score indicates low level of depression. Her anxiety was assessed on a scale of 0-10 through objective measurement questions as well. She scored an 8.

Perinatal Mood and Anxiety Disorders
For this session, focus on solution focused brief therapy, narrative, and/or feminist therapy modalities to engage with the client.
Solution Focused Brief Therapy: This type of therapy focuses on the here and now and breaks down each issue into small steps where solutions can be identified and implemented. It is a goal-directed and collaborative approach to change that is performed through observation of a client’s responses to a series of precisely constructed questions.
Narrative Therapy: This type of therapy allows the client to tell a story then reflect.
Feminist Therapy: This type of therapy focuses on societal, cultural, and political causes and solutions to issues faced by women or minority groups.
A very depressed 36-year-old African American woman named Mindy is referred to outpatient therapy five months after giving birth to her third child. She states she has two boys and the newest is a girl. She talks with a flat affect when describing her lack of interest in being a mother. She also expresses concern over recurring thoughts of seeing herself drop her baby, and cries as she discusses it, stating, “I would never hurt my baby. Why do I keep having images of dropping her down the stairs or in the bathtub? It is so distressing.” It has been difficult for her to get out of bed in the morning, and showering/hygiene has been poor. She presents to the appointment wearing leggings, an oversized shirt, very messy hair, and house slippers. She reports her husband is upset because he wants things to get back to "normal." She also expresses fear of letting others hold the baby, as she has similar images of dropping the baby during transfer to the other person.
She reports no complications during pregnancy or delivery, but states she was much more emotional this pregnancy but is unsure why. She reports she is breastfeeding and that this is the only piece of motherhood she feels she is coping well with. She cries more as she discusses missing her life before the third baby, and how her other two children were such good babies. She states, “This one . . . she just never sleeps. I can’t get any rest. I am so tired.” She struggles a great deal to transition to taking care of three children versus two and expresses she is a failure and things are never going to be “normal” again. She has decided not to return to her job as a financial advisor and, instead, has decided to stay home full time.
Her husband does accompany her to the clinic, but she reports to the session alone, leaving him and the baby in the waiting room.

The purpose of this assignment is for you to complete a comprehensive psychiatric mental health assessment of a child/adolescent as practice for entry into PMHNP clinical practice and continued growth in interviewing skills throughout the term.

This should NOT be a patient you have encountered in your work but, instead, should be a family member or friend (who gives consent). You are seeing a new client but have completed the majority of the initial interview. As you wrap up, you will move through some psychotherapy utilizing motivational interviewing to collect the remainder of the data.
Motivational interviewing (MI) practitioners are similar to guides; that is, someone who does not choose the destination but offers information about the paths the client might choose. The evoking (immediacy) phase of MI focuses on recognizing and reinforcing change talk and eliciting and strengthening change talk. It utilizes techniques for rolling with resistance. This week, you will be introduced to question-asking techniques that will elicit and clarify change talk using the OARS and DARN- CAT techniques.
You will be applying these techniques in the video recording this week and will discuss them in your peer and self-reflection.
Case Study
Because a patient, 36-year-old Hispanic-American male, Kevin , has been showing signs of addiction to Xanax, the primary care provider decides to discontinue the patient's Xanax prescription and refer for psychotherapy. The patient responds with anger, pleading that he can't function without the medication.
As you move through the initial assessment, the patient states that he had a back injury on the job. He works for a concrete company and fell into a prepped basement area before the concrete was poured. He had some pain meds during recovery and was given Xanax for increased anxiety. He states he enjoys his job very much and that he cannot lose it as he is the sole provider for his family.
He lives with his wife and twin boys, age eight. He stresses again that he works hard for his family and can’t afford to miss work due to anxiety so he needs the Xanax to function. He denies any substance abuse or past medical history other than the back injury eight months prior. He has no legal history and no military history. His extended family lives near, and he reports a pleasant childhood with continued support even still.

You are seeing a current client for a follow-up session with new developments after a recent loss. This is not an initial interview. For this session, focus on the interpersonal therapy modality to engage with the client
Interpersonal therapy –?This type of therapy focuses on relationships with other people and life transitions. It is based on the foundation that relationships are at the center of psychological problems.
Case Study: An isolated 70-year-old Asian man, Raymond, lost his wife six months ago. Though he has long disliked people, he now finds himself suffering "unbearable" loneliness. He states he hasn't considered suicide but has increased his alcohol consumption. He notes typically drinking "a six-pack every evening in order to stop thinking so that I can fall asleep." Before his wife passed, he had been sober for 20 years. He states he just feels like he is withering away and that he feels like he is almost dead.
Mr. Zing also remarks that, independent of his dizziness symptoms, he feels unsteady on his feet when walking. He has started using a cane but doesn’t like to use it inside. When asked about previous falls, he says he hasn’t fallen. However, he says his elderly neighbor recently fell and is now in a nursing home. Now he’s fearful about falling and becoming a burden to his family. Although Mr. Zing has spinal stenosis, a recent steroid injection has relieved severe low back pain. For this assignment, you will present the topic to the rest of the class in the form of a PowerPoint poster. This is one slide, in the format of a poster. A second slide will be used for references only. Get creative!
You should have already picked your topic from the list of therapy modalities in Week 4 Activity 2: Individual Poster Presentation Topic Choice. No more than two people can reserve the same topic.
The presentation must include a brief background/overview of the topic and reference a minimum of two evidence-based articles used to provide treatment guidelines/recommendations. Please utilize the poster presentation template (PPT) to get started.
