Week 7 Discussion
Clinical Case Study
This week's discussion addresses dermatological disorders. Please review the clinical case study below and answer the questions that follow:
Clinical Case Scenario:
A 7-month-old boy presents with an erythematous, confluent, slightly raised, and scaly rash on his cheeks; and his extremities are also covered with a fine papular rash. The infant has had some scaling behind the ears and on the scalp since early infancy, but the symptoms have recently increased. The mother applies baby oil to the scalp to relieve scaliness. Except for some intermittent rhinorrhea, the infant has otherwise been well. Immunizations are deficient; he received only the first set when he was two months old. The family history is positive for bronchitis. The infant's weight is at the 75th percentile, and the height is at the 50th percentile. Vital signs are normal. The physical examination is normal, except for the presence of the rash.
Questions
What are the characteristics of papulosquamous eruptions? Be thorough and descriptive.
What are the common conditions associated with papulosquamous eruptions in children? List at least 3 common conditions and include the pathophysiology of each condition.
What are the appropriate treatments for common papulosquamous eruptions? Why?
When should children with papulosquamous eruptions be referred to a dermatologist?
Define the following: a) confluent, b) papular, c) papulosquamous, and d) rhinorrhea

Clinical Case Scenario
An 11-month-old girl, well-known to your clinic since birth, arrives for a well-child visit. The parents report that their baby drags her legs rather than crawling on both hands and legs. They are concerned that this is abnormal. The father says that the child only recently began crawling and she does not pull to a stand. You noted at her 6-month visit that she was not rolling over or sitting; previous visits were unremarkable as was the mother’s pregnancy and vaginal delivery. While examining the baby, you note that when she is held by the axillae her legs are extended and crossed (i.e. the “scissoring” posture).
What is the most likely diagnosis? Why?
What would be three differentials in this case? Why?
What developmental milestones are expected at this age?
What do you believe is your best course of action/treatment plan for this appointment? Why?
NSG6435 Practicum III Family Health Paediatrics
Week 9 Discussion

He has no history of trauma, fever, or joint swelling. The family history is negative for rheumatic or collagen vascular disease. The boy's height and weight are at the 50th percentile for age, he is afebrile, and the physical examination is unremarkable.
Questions
What are 3 differential diagnoses of leg pains in school-age children? Include the pathophysiology of the 3 differentials.
What laboratory or radiographic studies are appropriate for children with leg pains? Explain
How do musculoskeletal injuries in children differ from those in adults? In terms of injury type and location.

Clinical Completion Attestation
Clinical Documentation Assignment
Clinical portfolios are maintained through CORE ELMS. Students are REQUIRED to enter data into CORE ELMS within seven (7) days of the clinical encounter. Students are expected to update their profile information, clinical schedule, time logs, and field encounters every week.
EXPECTATIONS:
1. It is the student’s responsibility to ensure that his/her preceptor has submitted a midterm and final student performance evaluation via CORE ELMS’ electronic form. Students should check CORE ELMS for the evaluation. If the evaluation has not been submitted then students are to respectfully and politely remind the preceptor to complete the evaluation. Both evaluations are REQUIRED.
2. Students are REQUIRED to have a total of 150 hours entered in Hours Tracking. The hours tracking dates must match the dates of the recorded field encounters.
3. Students are REQUIRED to enter a field encounter for every patient seen during the clinical rotation within 7 days of the encounter. Students are REQUIRED to have a total of 9,000 minutes of direct patient (Direct Patient Time = Time with Patient + Consult Time with Preceptor).
4. Students are REQUIRED to have the verification of hours form signed by their preceptor for every clinical day completed. The form must be completed in its entirety (tally the hours). The signed form must be uploaded to hours tracking in CORE ELMS by the student.
You are to attend every practicum experience as scheduled with your preceptor and document ALL patient encounters in CORE ELMS. In order to stay current, it is best to update your CORE ELMS logs after every clinical day. All patient encounters and time logs must be logged within 7 days of accomplishing them or they will not count for your practicum time. There is a minimum of 150 hours and 9,000 minutes of field-encounter entries required for successful completion of this course and these must be entered by end of the course for a passing final grade.
Summary of REQUIRED Clinical Documentation in CORE ELMS:
150 clinical hours logged in hours tracking
9,000 minutes of direct patient time logged in field encounters
Midterm and passing Final student performance evaluation completed by the Preceptor
Verification of Hours Log signed and uploaded in hours tracking
Evaluation of the Preceptor completed by the student
