Week 2 Assignment
SWOT ANALYSIS
A SWOT analysis is a tool used in gathering and identifying assets and resources of a community along with strengths and weaknesses of the environment. A SWOT analysis allows for clear and succinct communication of the four main components of the acronym itself: strength, weakness, opportunity, and threat. This tool provides an organized and systematic way of gathering the information collected from a community, in order to determine the areas of change that are possible and explore new and alternative solutions to issues within a community.
For this Assignment, refer to this week's readings, paying particular attention to the Examples and Tools sections of the resource SWOT Analysis: Strengths, Weaknesses, Opportunities, and Threats. Then conduct a SWOT analysis of a community in an area or neighborhood. Use the questions in the Tools section of SWOT Analysis: Strengths, Weaknesses, Opportunities, and Threats for directions toward conducting the SWOT analysis.
Assignment (2–3 pages)
Write a paper addressing the following:
Provide a summary of the information you found from your SWOT analysis.
Explain your reason for choosing this particular community.
Explain the most important strengths, weaknesses, opportunities, and threats, you found about this community in terms of addressing its health issues.
Explain the information that you found to be most surprising about the community. Also, explain how you can use this information in your community health needs assessment.

Week 3 Assignment 1
WALKING OR WINDSHIELD SURVEY
Walking surveys are systematic observations made while walking through the chosen community. Windshield surveys are systematic observations for data collection while being in a moving vehicle ("Windshield and Walking Surveys," 2013). Windshield or Walking surveys provide you an objective viewpoint of the surveyed community. This, in turn, helps you to accurately assess a community's health needs. You can use either type of survey to invite and mobilize the community to participate in programs. There are times that community members do not realize the number of assets and/or resources their community possesses. A windshield or walking survey can bring to light such assets or resources.
For this Assignment:
Consider your own community or select a community you are familiar with, for conducting a walking or a windshield survey.
Read this week's Learning Resources and pay specific attention to the resources concerning walking and windshield surveys.
Review the resource Community Tool Box Chapter 3-Section 21: Windshield and Walking Surveys from this week's resources.
Decide on the areas you will include in your walking or windshield survey.
Decide when you will conduct your walking or windshield survey.
Develop a questionnaire for your walking or windshield survey using the questions from the resource Community Tool Box Chapter 3-Section 21: Windshield and Walking Surveys (What to examine in a general community assessment survey). You can also generate additional questions, as you seem appropriate.
Assignment (3-4pages)
Conduct a walking or a windshield survey. Submit a summary of your survey findings. Attach the questionnaire with your survey findings.
After conducting this walking or windshield survey, explain what other methods you think would be helpful to collect primary data from this community and why.
List other questions you would ask in a walking or a windshield survey.
Note: Please remember the following suggestions while conducting your survey:
Prior to conducting your survey, make a checklist to ensure that you address all your questions, and observe all the areas you want to.
Try to be unobtrusive.
Carry identification.
Record your findings as you go.
Pay attention to safety.

Week 3 Assignment 2
COMMUNITY HEALTH NEEDS ASSESSMENT
A community health needs assessment involves a number of steps. As a health educator, you need to be able to complete each step efficiently in order to produce an accurate assessment of the community's health needs. The starting point of a community health needs assessment is to know why you are conducting the assessment and how you are going to use the assessment results. Once you thoroughly understand the purpose of a community health needs assessment, you will be able to conduct the assessment and utilize its results in a more efficient and fruitful manner.
Read this week's resources. Then, choose a community you would like to focus on for this assignment. This can be the community you reside in or a community with which you are familiar. Select a population (e.g., elderly people, teenage mothers) that you want to focus on within the community.
The Assignment (3–4 pages excluding APA citations)
Address the following:
1. Present the purpose of a community health needs assessment:
For this section, clearly articulate your understanding of what a community health needs assessment is and why someone would conduct one. Explain your understanding based on the resources, and apply this information to your chosen community
2. Describe your community and include details about the population within the community:
Using the Understanding and Describing the Community resource from this week’s Learning Resources for guidance, provide a description of the community you selected.
Identify two primary data sources you would use to learn more about the community. Explain why you chose these methods.
3. Consider that you are being asked to complete a community health needs assessment in this community. What goals would be set?
Explain three overarching goals for a community health needs assessment.
4. Review this week’s Learning Resources on the Social Determinants of Health.
Using your familiarity of the community and resources to support, share your initial concerns relative to the Social Determinants of Health and their interaction with health outcomes in this community. Which Social Determinants (e.g. education quality, health care access) are prominent? Suggest two strategies for integrating a Social Determinants of Health focus in a community health needs assessment.

Week 6 Assignment
DATA COLLECTION MEASURES
Data collections methods for community health needs assessment include quantitative and qualitative data collection methods. The goal is to collect data about behaviors, beliefs, concerns, resources needs and to uncover contexts behind the observed health challenges (University of Kansas, 2023). In this assignment, you will examine the quantitative and qualitative data collections methods used for community health needs assessment. Also, you will consider the importance of evaluating resources in a community.
Address the following two areas:
1. Choose two quantitative and two qualitative data collection methods. For all methods chosen, please discuss the following:
Explain how the method is generally used in community health needs assessment.
Describe the strengths and weaknesses of each method.
2. When evaluating resources in communities, health educators are reviewing needs through the lens of the Social Determinants of Health. Using the Learning Resources from the week, describe three resource needs common in communities challenged by the Social Determinants of Health. Explain what the best methods would be for collecting data about these lacking resources.

Week 8 Assignment
UNDERSTANDING THE ROLE OF INFORMATION DISSEMINATION AND THE USE OF DIGITAL TECHNOLOGY
Regardless of the method one chooses to use to disseminate findings from a community health needs assessment, the information reported needs to be adapted to the target audience. Digital technology has been used as a tool to support the dissemination of information across different disciplines, including health communication. It is important to continue exploring the uses of digital technology to support community engagement during information sharing.
Because a needs assessment targets the community in which data are collected and shared, it is important to consider how information from the assessment is disseminated and who is involved in the information dissemination. Modifications might be necessary based on the target audience and the results of the community health needs assessment.
Assignment( 3-4 pages)
For this Assignment, review the information dissemination strategies and digital technology content in this week's resources.
Address the following in context to the resources provided;
Describe the purpose and intent of dissemination as a step in Community Health Needs Assessment.
Describe the different types of information from a community health needs assessment that must be disseminated. How do we determine who will receive the disseminated information (who is the audience)? Also, consider any modifications to the disseminated information that might be necessary depending on the audience?
What are some of the most common dissemination methods used for community health needs assessment?
Describe how digital technology can be used to disseminate Community Health Needs Assessment data and information. Discuss three digital technology modalities that can be used and describe the advantages and disadvantages of each.

Week 10 Assignment
PROPOSAL FOR CHANGE
Social change remains at the center of what health educators seek to accomplish. Social change in health education relates to tangible improvements in practices, education, outcomes, policies, and ultimately, lifestyles toward better wellbeing. As we continue to evaluate the Social Determinants of Health and their interactions with community health outcomes, we are challenged to not only investigate root causes but devise meaningful solutions. This week, you will evaluate proposals for change and consider how they are used to support improvements in communities after a community health needs assessment is completed.
A proposal for change is an advocacy tool that allows a health promotion team to develop a plan of action to address a health issue in a community. It goes farther than simply identifying the problem; the proposal presents action alternatives that can lead to social change. Armed with the insight from the community health needs assessment, health educators can use the proposal for change to serve as a great first step to verbalize the unified course of action and a way forward. For example, while the community health needs assessment might have identified lack of transportation serving the elderly in rural areas to assist with healthcare access, the proposal for change would advocate for a fully funded bi-weekly transport van run by a team of trained community volunteers to fill the gap of this need. This week, you will develop a proposal for change using the Proposal for Change learning resources.
Assignment 3-4 pages:
In week 8, you developed a coalition that could address a specific health issue in your community. With that health issue and coalition’s goal in mind, this week, you will develop a proposal for change that will represent solutions to the health issue.
Provide a brief recap overview of the health issue and the goal of coalition you developed in week 8. Do not copy and paste from the week 8 assignment (this is self-plagiarism). Rather, paraphrase as needed and cite where required.
Describe the purpose of your proposal for change. What is the coalition’s goal and why are you advocating for change?
What are the key actions you are proposing to address the health issue? Keep in mind that these should be tangible actions that could take place in the community with descriptions of how to accomplish them. Identify 3-5 key actions that are attainable. You must back up these suggestions with referenced support. Plan to research how communities like yours are addressing the health issue. Be sure to review what works and what does not so that you can justify your proposed actions.
Discuss why these key actions will lead to social change in your community. Consider major aspects such as health equity, social justice, and human rights.
