Week 1 Discussion

Risk Stratification

Risk stratification is an important technique that allows patients to be classified according to their health risk status, taking into consideration many factors, such as diagnosis, age, BMI, comorbidities, labs and other assessment scores, health behaviors and health literacy, and social and caregiver support needs, to name a few. Utilizing such a framework or model can be used not only to identify patient-specific risks to refine treatment plants, but can also be applied to improve workflows, better manage population health, and effectively use resources.

For this Discussion, you will consider risk stratification in the preoperative environment.

To prepare:

Review the risk stratification video in this week’s Learning Resources.

You will receive a set of patient scenarios from your Instructor. Review each of the three patient scenarios provided. Identify each patient as high, intermediate, or low risk.

Consider patient education needs and strategies for each patient, as well as what the informed consent for each procedure would be.

Group A

A 60-year-old female with no previous cardiac history, except for preoperative for stratification for a new murmur, is sent to you. An echocardiogram is performed demonstrating an ejection fraction of 60%, and severe aortic stenosis. Her proposed surgery is a total knee replacement.

Group B

A 25-year-old male is sent to you for preoperative risk stratification. His proposed surgery is an emergency cholecystectomy. He is active and has no exertional symptoms playing basketball for over an hour 3 times weekly. He has no previous cardiac, medical, or surgical history.

Group C

A 75-year-old female with a history of coronary artery disease with previous CABG and PCI, hypertension, and hyperlipidemia is sent to you for preoperative risk stratification. Her proposed surgery is hip replacement. You are unable to assess her functional status due to hip pain, which renders her mobility challenged. Her previous echocardiogram demonstrates an ejection fraction of 55–60% with no wall motion abnormality. She has no active anginal or exertional symptoms.

Week 2 I-Human Case Study

Assessing, Diagnosing, and Treating Patients With Acute Musculoskeletal Disorders

Musculoskeletal/orthopedic conditions often affect patients’ function and mobility. Changes in function and mobility sometimes are subtle and might not be identified immediately, requiring you to monitor patients closely for signs and symptoms. For this Assignment, as you examine this week’s i-Human patient case study, consider how you might evaluate and treat patients who present with orthopedic conditions.

To prepare:

Review this week’s Learning Resources. Consider how to assess, diagnose, and treat patients with acute musculoskeletal conditions.

Review this week’s i-Human case study. Based on the provided patient information, think about the health history you would need to collect from the patient.

Consider what physical exams and diagnostic tests would be appropriate to gather more information about the patient's condition. Reflect on how the results would be used to make a diagnosis.

Identify three to five possible conditions that may be considered in a differential diagnosis for the patient.

Consider the patient’s diagnosis. Think about clinical guidelines that might support this diagnosis.

Develop a treatment plan for the patient that includes health promotion and patient education strategies for patients with acute musculoskeletal/orthopedic conditions.

Week 3 I-Human Case Study

Assessing, Diagnosing, and Treating Patients With Immunologic, Autoimmune, and Transplant-Related Conditions

Immunologic, autoimmune, and organ transplant patients need continuous, long-term care. In comparison to healthier patient populations, they are more fragile and require very individualized care plans. For this Discussion, you explore special considerations when assessing, diagnosing, and treating these patients.

Week 8 I-Human Case Study

Assessing, Diagnosing, and Treating Patients With Integument Conditions

J.V., a 50-year-old patient with history of eczema presents to you complaining of lesions on the right side of her face and neck. She thinks it is a flare-up of her eczema and is asking for a refill of her ointment, TAC 0.1%.

She complains of some “itching” and a bit of “tingling and pain” to the lesions. She’s a pharmaceutical worker and thinks that the pain maybe due to contaminant exposure, and denies any other associated symptoms.

A 46-year-old male presents to the office complaining of a pruritic skin rash that has been present for a few weeks. He initially noted the rash on his feet, but it then spread to between the fingers, his wrist, and waist. He notes that it does not seem to be on his face or trunk. He recently came home from a trip to Florida where he had stayed in multiple hotels. He takes occasional ibuprofen for knee pain, but denies taking other medications or having other health problems. He has no known drug allergies. The physical examination reveals several tiny vesicles and scales in between the fingers, on the feet and ankles, around the patient’s wrist and around the belt line.

How would you assess, diagnose, and treat these patients? For this assignment, you examine an integumentary patient case in i-Human.

To prepare:

Review this week’s Learning Resources. With integument conditions in mind, consider how to assess, diagnose, and treat patients in acute care.

Week 9 I-Human Case Study

Assessing, Diagnosing, and Treating Patients With Ectopic Pregnancies and Sexually Transmitted Infections

The Centers for Disease Control and Prevention estimates that there are 20 million new cases of sexually transmitted infections every year in the United States (CDC, 2017). STIs may present serious health implications for infected patients—especially for those who are unaware of their health condition. Although they may occur at any age, STIs disproportionately affect younger adults ages 15–24, who account for more than half of all new infections (CDC, 2017). Studies also show that women are at greater risk of contracting these infections, and also tend to have more severe health problems resulting from infections than men (U.S. Department of Health and Human Services, 2009). As an advanced practice nurse, it is important to emphasize the importance of prevention and STI testing for all individuals, regardless of age, sex, marital status, race, ethnicity, or socioeconomic status.
